My guess would be someone falling on their head, and the subsequent post-crash impact.

Dura Ace levers don't break. That's what it said in my owner's manual.

the tape is all ripped up, and there appears to be blood. My guess is a crash?

I inherited the other side. Kid bought a bike and a month later crashed it 'just wrong' and broke the end off the lever

Shimano USA doesn't import the part to fix it either.

Brake doesn't look all that clean. I see some smudges. And on the bar wrap.

Funny you should say that. Way back in the day I was helping assemble pro team bikes and we had the first generation of Campy carbon levers. We thought they were so cool.....at least until we installed them.

You'd mount them on the bars, attach the cable, give it a good pull to set and stretch things, and hear a loud snap right before the lower 1/2 of the lever rattled off the floor. All of them broke. A good 6-8 pair and we had to have aluminum versions red-labed in to get the bikes out.

Some times things don't work.
