It is somewhat variable based on the level of detail (open face plate, custom lugs, etc).
My most popular stems are quill and thread less fillet brazed jobs with single bolt faces, chromed (in one of an assortment of finishes). Those are regularly in the $400 range. There are a boat load of "standard" options, and even more one off variables. It's best to ask for a specific project. (Integrated hanger)  (3D graphic for a Monster Fat)  (shifty bits on a retro-reproduction quill)  (double length, single height adaptive stem)

I'm totally intrigued by that double length, single height adaptive stem. Is that adaptive for a ride with two widely varying arm lengths?

Yes indeed. It was made for a gentleman that has a 2" discrepancy as well as some significant hand and grip issues. The stem allows him to have even arm extension and a comfortable weight on both hands. It also allowed him to use his front shifter and brake for the first time.
The stem extensions are at compound angles that put the bar sides parallel and at the same height, but at different reaches. That was a fun project.
