qr full-carbon gravel fork 700x45 / 650b 2.1 ?

Does anyone know who made such a fork?

Steerer has a slight crack; chasing a tapered replacement.

Qr + carbon steerer makes it tough. Are you sure you can't convert your front hub to TA?

The CX should. As long as the taper diameter and axle-to-crown measurements are good it sounds like a solid match.

It's advertised as 650x 2.0, but I know I've seen bigger in there, at least for the TA version.
