What about Chris King Gripnut? Supposed to work with 33-38mm?

As potato said... you can go facing the top of the front tube, other solution is to remove the washer... another solution is to sand/file the top nut at the bottom or swap the top nut for something else. Or file the top nut and the top cone of the headset (the piece that goes under the top nut), thing ive seen before many years ago. Is aluminum, nobody will know :P

Dura ace and 105 sc are nice headsets, specially the 105 sc... super tiny headset, still remember when I saw one the 1st time.

I have two bikes with threaded forks and headsets with Campy Chorus headsets running with "No" washer. No problems with headset operation or loosening up. In my case "No washer" = no problem. YMMV.
