The
Sheldon Brown setup has always worked for me (though I do have a Campy BB cup somewhere in my basement which is permanently affixed to the tool. Good news is that it's not in the frame...).
In addition, I use a 24" breaker bar. Put the frame on a rug, stand on the breaker bar. This has worked in cases where the vise has not.
