Looks like an imprecise, noise-making nightmare.
I think I'd buy a bike with a different head tube before using that.

That's pretty off base, offset bushings are pretty common on everything that needs precise alignment. How do you think your car is aligned?

Only thing is you have to run a 1.125" headtube on a 1.5" headset...so right now, only applicable to mountain bikes.

Interesting. The Ritchey Annapurna years ago used to have an adjustable HTA.

1) In a way that requires laser aiming in a pro wheel shop. ;)
2) In a way that gets screwed up when I hit a pothole. :crap:

Here is the first example of a product that clever Mr. Bedford clearly anticipated when he decided to start building 1.5" head tubes with 1 1/8 inserts pressed in. Along with many other uses only the future can predict :)

I just ran across the product when reviewing headset how to videos on Cane Creek's website. Surely there must be some demand for it or Cane Creek would not have went to the trouble developing it? Why would anyone want to change the head tube angle anyway? Would it help old frames accomodate modern forks? Handling?

For MTBs this allows you to change the HTA to match changes in fork travel..

Run long travel + slacker angle for a course with lots of steep downhills.

Run shorter travel + steeper angle for a relatively flat XC race.

I might have the combinations backwards..

Not sure why you'd want this on a road bike.
