I think the above are mostly threadless. Threaded custom stems is a good question. I would imagine you could get most custom frame builders to build you something.

Dave Kirk's website says he makes them
Sacha at Vanilla makes nice ones (I have one on my Vanilla)

The former has a picture of a threadless, while I have a threaded one of the latter. I would guess that each can make either, however.

Try TiCycles.com out of Seattle. They made a Ti 1 incher (threadless) for me. Clean job. I can recommend them. Jeff N.
