Question: Is the raised line on the sides of the cleat the actual center of the cleat or the theoretical axle center? Does it have anything to do with the location of the axle?

nope.... the notch or line on the cleat represents the location of the pedal axle. ball of the foot in the shoe is referenced from the notch.

Uhm...I think I just said that. Ball of foot goes over line, line represents axle, ball of foot over axle, ie; purpose of line. I was not really asking. :)

I was most sure it was the axle placement. I have been messing w/ my DA cleats to better fit carbon soles. I found if I wedge or shim the front (between cleat & shoe) I get less binding and an easier rotation.
