Robin Mather in Bath, England will build them - anything from a burly fillet-brazed ahead to a slender lugged and chromed 1" quill model.

What sort of stem are you interested in? Threaded or threadless?
I have a Richard Moon threaded stem on my Ti Spectrum. It's as pretty in real life as it is pictured on his web site: http://www.cwo.com/~lunarlab/stem.htm
Johnny Coast makes a beautiful threaded custom stem, too. However, it's only available with a new frame, and it's $400. That's enough to make a Technomic Deluxe or a Pearl look really, really good...

as you can see, there is a lot of choice and then some. methinks you could get much more specific direction towards the builders that would be appropriate for your needs if you disclose the frame and/or type of build for which the stem is intended. :)
