Depending on your needs, most custom steel builders who do custom stems will likely make a threadless stem w/ integrated spacer.

On some bikes a matched custom stem looks great....but call me crazy, does it seem that there is a craze of everyone and his brother doing custom color-matched stems? Something about the look, poorly executed, reminds me of cheap early mountain bikes.
