Are free fax apps safe to use? Any recommendations would be great.

I have used Turbofax without problems. Don't know about level of security, though.

It's so much easier to scan and email. You might take a look at https://readdle.com/scannerpro if you have an iPhone, and there are Android equivalents. Fast, easy, and you don't even need a scanner machine.

Are you in an industry that requires faxing?

Faxplus worked okay for me. Free to send but subscription to receive.

Just so happens I am dealing with a government agency and need to fax 1 item.

Many people use fax for documents. I think modern apps are supposed to be secure, but you can read their privacy terms and conditions first. I use the free mobile fax app (https://technicalexplore.com/best-free-fax-app-for-android/) and have never had any problems.
But even if you use a paid fax app, it's a small price for convenience and quality. It's time to stop using the fax app of the last century and step into the age of online fax. It seems pretty silly to me to be tied to an office today. Many employees need to travel somewhere for work, and others work from home.

In iOS you can scan from the Notes app (open a note, tap the camera icon, select scan).

I just recently learned this and it is awesome. Scan to pdf and way quicker than using my desktop scanner.
