The hand cycle is incredibly efficient. The clear winner here ;)

The fixed rider must go through a lot of tires.

Bit surprising that the hand cycles can't get more of an edge through the corners vs a fixed gear.

Cables would be a mess using 180* offset cranks, plus your arms are not as strong as legs so working in tandem, like rowing, is more efficient. You also have steering to do with the cranks and hands together is easier there as well.
