Klein headset help

My nos Klein Q pro xx is all finished, except i am having a frustrating problem with the headset coming loose.

I'm doing everything correctly regarding starnut, steerer tube 3mm below stem, then i tighten it up and it feels fine...go out the driveway for test ride of 30 yards and it comes loose again.

i'm thinking it is related to this bearing pictured on top of head tube. Does it need to be "pressed in" with the proper tool? Right now i just fit it in by hand.

Yes, the top headset needs to be pressed in. Follow directions on page 67 in the link I provided in your thread about internal cable routing.
