Anyone ever use a metatarsal pad successfully to alleviate/ eliminate hot spots?

I have them in my orthotics...

I have mixed feelings about them, but they have worked for me.

basically they act to spread the toes and other foot bones.

If you have high insteps, (the top of your foot), the added volume from these can be too much for some shoes.

Of the insoles I've used, the specialized with the metatarsal pad are the most comfortable. I have considerably less pain along the ball of my foot with them, as compared to insoles with just as much arch support, but no bump/pad in the forefoot.

So, I guess I'm a fan.

I use them. I make my own and I do not place them that far back toward the arch.

Yes, in both cycling shoes and my work shoes...bought these.

Not a pad but whole insert..clinched toes...this pad eliminated that and hot spots under foot.

I've been using them in one pair of cycling shoes with custom orthotics off and on for 2 months. Some days they seem to help some days not. I also alternate with another pair of cycling shoes that doesn't have them and I rarely get "hotfoot".
I think my particular case of off and on "hotfoot" may be the result of super stiff carbon fiber soles. I didn't have a problem till I switched to them and when I wear the older pair of stiff but not super stiff shoes I don't seem to have "hotfoot" as often, if at all. Alas,it's all anecdotal not very scientific.

they've made a big difference for me...no more numb toes or hotspots. Recommend you give them a try...$30 well spent

I have been using these Hapad Metatarsal pads in my walking and cycling for years and they work great, are long lasting and give me comfort. Check them out.
