Have been taking some meds prescribed by my primary care doc, and I'm trying to figure out if the next step is to see a specialist or to stay the course. PM me please.

Yes, dartmouth does have a med school. But I wouldn't trust those students ;)

More seriously, Dartmouth Hitchcock Medical Center (the local hospital) has been running wait times of upto 2 months to get into see a dermatologist. So, if I were to decide to see someone, I'd likely go down to Boston as they seem to have shorter wait times.

I didn't want to post too much info here, as I traditionally do not put too much faith in internet diagnoses by non-experts. I have been off the bike completely for 2 weeks and really cut my riding down during the last 4. First because I thought I had jock itch, tried the usual lotrimin which has worked in the past, but that wasn't effective. Saw my PCP, he wasn't convinced it was fungal, but gave me some Sporanox, no real improvement. Now I'm using a cream that is Nystatin and triamcinolone. Slight improvement, but still not healed.

Trying to figure out if I'm headed in the right direction or what else this could be.

Many should pay attention to this.
Often times a rider will blame a saddle or bibs for what is actually a fungus infection.
Fungus likes to grow down there. Even if hygiene is good.

I had a similar problem. Under my armpits though. Went to a Derm. Told him I was a cyclist.

Complete idiot that he was, told me condescendingly that I should shower more. Then gave me a prescription for an anti fungal.

Turns out (of course) it wasn't fungal or related to showering. It was psoriasis. Summer riding, tight, wet, jersey pulling and rubbing. Body counter attacks with psoriasis.

New Derm, prescription for some type of hydrocortisone. Apply and a few days later gone, never came back.

My point is that it doesn't have to be jock itch.

That's what I have and it sucks really really bad. Consider your self lucky that it had not come back but. If I had to guess I'd say you are young. When I was in my 20's and first got it it would go away for a long time now it's permanent. Just have to deal with it. Many more have it a lot worse than I do so I try not to complain but it sucks :(

Last summer, I was convinced I had shingles due to a nasty rash in the right place to be shingles. Turns out it was an abrasion from my gloves wiping away the sweat.

I know I'm a non-expert, but I think I would try an OTC cortisone cream while you are waiting to see the dermatologist. The PA I went to see about the rash gave me some, she said if it was shingles I would know soon enough and she was right. She thought it was probably a heat rash, which was pretty close. But that's when I figured out it wasn't even a rash and stopped the cream because I reserve cortisone for stuff I can't cure by leaving it alone.

WOW...I'm in the very same boat. If I survive this I'll put up a complete thread...but bottom line; I wore my shorts several times without washing them and left them lying on the bathroom floor. I am 6 weeks into a horrible rash on my scrotum and penis. HORRIBLE.

1. Tried everything over the counter imaginable...no change for two weeks
2. Went to the Dr. and he prescribed Clotrimazole and Betamethasone Dipropionate. Two weeks, no change except a bit less itching.
3. Went to dermatoligist and he thought it might be fungal but wasn't sure. I started samples of Oxistat. $300 for the prescription and happy to have a lot of samples. Seemed to help the first few days and then no change except I started to look real bad....I won't go into details but wondered if it might fall off?
4. First Doc (my Primary) reccomended Ketoconazole. Took for ten days with no change.
5. Back to the specialist and he has now given me Protopic to apply in the morning and Naftin to take at night. Does't seem to be doing much, but not worse either.

I do notice that everything down there (and I mean EVERYTHING) is much less red in the morning and almost purple in the evening before bed. Don't know what that means? I'm told if this doesn't work they will have to take a biopsy....arrgh! When I asked what they would be looking for he told me there is a rare condition in which the nerves get all screwy and they have to take things to the next level. I'm hoping that doesn't mean amputation. :)

This is all my fault, I was a lazy slob and now I'm paying for it. Ain't fun at all and Matt I hope you get better soon.

PS: Yes it has been six weeks since I've "gotten friendly" with my wife.

Today I saw a girl with a Dartmouth t-shirt give a dirty look to an older kid (27ish) who was wearing a Johns Hopkins t-shirt. Is there a vicious rivalry between their two med schools? Is it possible that there is bitter resentment because there are more Johns Hopkins grads who are board certified surgeons? Or is one campus just prettier than the next. This stuff is more interesting to me than the BCS. Real talk!

Well, first I don't sweat much. Second, I have three pairs of shorts that I rotate and eventually some get washed right away and some don't as I get confused as to which are which. Now I'm washing them every time and same day. By the way, been riding since '73 and this is the first time anything like this has happened...live and learn. Assuming you live.

If it's really jock itch - tinea cruris - then a topical antifungal would be the first course of treatment, and either a stronger topical or oral antifungal next.

Could also be candidiasis, commonly referred to as a yeast infection. Normally skin blocks yeast, but any area where there is abrasion / irritation allows the yeast to penetrate the skin barrier. Like cycling in hot weather or less than perfectly fitting clothing.

There are a few differential diagnoses - folliculitis (could be bacterial), contact dermatitis, psoriasis, seborrheic dermatitis, intertrigo. A secondary bacterial infection that co-exists with a primary fungal infection is not out of the realm of possibility.

Derm consult is your best bet. I'd look for a derm person affiliated with a major medical center if at all possible.
