I will be in San Francisco next week and need to rent a road bike (54cm) in order to do a ride with a friend. Any advice on which shops in downtown do rentals?

Seconding Pacific. I have recommended it to a few visiting friends and they have reported back with good experienced.

City Cycle (http://citycycle.com/articles/rentals-pg144.htm) rents out Trek Domane bikes

The shops along Stanyan st by Haight Ashbury may be worth a look as well.

Not in SF downtown, but there's a relatively new bike rental shop called the Sausalito Bicycle Company that rents out Bianchi, Colnago, Giant Look, Marin and Scott. Check it out:

The last time I was in S.F., I rented a Giant from Sausalito Bicycle Company.
The rates were a bit high compared to what I'm used to but, it is California...

The owner was there and he and his wife were very helpful with sizing, installing pedals, giving me a spares kit, etc...

I would recommend them.
