I think this all means you put the chain on the (U.S.) fifth cog and then use the barrel adjuster to line up the chain/pulleys with the (U.S.) fifth cog, full stop. It has nothing to do with clicks or some convoluted way of adjusting the barrel to get the chain aligned over (what we'd call) the fourth cog.

Makes perfect sense . . . .
