I think it's confusing, but the Italian and English versions say the same thing when they refer to both the "fifth" and "fourth" cogs. The only way this makes sense to me is to interpret it the way that Italian usage labels the floors of a building (like on the buttons in an elevator). U.S. usage would call the ground floor the "first" floor and so the "fifth" floor would be the one we'd be used to here. Italian usage calls the ground floor the zero or ground floor, making our fifth floor their "fourth" floor.
Maybe this is how Italian refers to all things that are stacked, except Sophia Loren, Monica Bellucci, et al.
I think this all means you put the chain on the (U.S.) fifth cog and then use the barrel adjuster to line up the chain/pulleys with the (U.S.) fifth cog, full stop. It has nothing to do with clicks or some convoluted way of adjusting the barrel to get the chain aligned over (what we'd call) the fourth cog.
Makes perfect sense . . . .
